JD Industrials (stock code 07618) has issued a reply form enabling registered shareholders to select their preferred method—electronic or printed—of receiving future corporate and actionable corporate communications.
Three alternatives are available:
1. Scan a personalized QR code to provide an email address for fully electronic delivery; no reply form return is required under this option.
2. Submit an email address in writing via the reply form to receive actionable corporate communications electronically.
3. Request printed copies of all corporate and actionable corporate communications for one year by ticking the designated box; this option supersedes any email registration.
Key procedural points include: • All joint shareholders must sign the reply form for Options 2 or 3 to be valid; unsigned or incorrectly completed forms are void. • If multiple email addresses are supplied, only the most recent will be registered. • Failure to supply a functional email address will result in printed delivery of actionable corporate communications. • Personal data provided will be used solely for dissemination purposes, may be transferred to the share registrar or subsidiaries as required, and can be accessed or amended in accordance with Hong Kong’s Personal Data (Privacy) Ordinance.
Completed forms should be returned to Computershare Hong Kong Investor Services Limited, 17M Floor, Hopewell Centre, 183 Queen’s Road East, Wan Chai, Hong Kong, using the enclosed postage-paid label for local mailing.
