rebates on longer GP consultations to be boosted by 40 per cent.“When people can’t afford longer GP appointments, complications and other health issues develop — people get sicker and need expensive hospital care,” Royal Australian College of GPs (RACGP) president Dr Michael Wright said.For Victorian mother-of-two Sarah Willits, 47, who has lived with kidney failure for more than a decade, managing her chronic health condition leaves no time in a standard session to discuss other health concerns.“By the time I get through all my physical issues with my GP, time is up,” Willits said.“I would love longer appointments to have the chance for a wellbeing check to get the help I need with related issues.”Dangerous effects of delayed carePeak bodies have said long consultations are sclerosis (MS) are invisible or subtle, and timely diagnosis depends on GPs having enough time to recognise symptoms, listen carefully, and refer early,” MS Australia CEO Rohan Greenland said.“After diagnosis, longer consultations are just as critical for managing comorbidities and the complex, lifelong needs of people living with MS.”Motor Neurone Disease (MND) Australia CEO Clare Sullivan said reaching an MND diagnosis is also “often clinically difficult” and that “access to longer consultations to provide rapid and accurate diagnosis is crucial”.Diabetes Australia CEO Justine Cain said for the two million people with diabetes, a long consultation can “provide critical time to consider a referral to a credentialled diabetes educator or endocrinologists where appropriate”.“You can’t always manage a complex chronic condition in a 10-minute conversation,” she said.The knock-on effects of delaying care and diagnoses can be drastic, including an increase in hospital and emergency presentations, Australian Patient Advocacy Alliance CEO Deidre Mackechnie said.“Inadequate management of a chronic condition can result in disease progression to the point of disability, including the need to access the NDIS,” Mackechnie said.Kidney Health Australia, Parkinsons Australia, Sexual and Reproductive Health Australia, and Dementia Australia, Palliative Care Australia are also among the 20 peak bodies advocating for long consultation funding.“Longer consultations with the GP will contribute to better management and consequently better outcomes,” the group said.“Analysis shows consults are easier to bulk bill, as bulk-billing incentives for GPs apply per eligible service, not per minute.This disincentivises longer session for GPs but there are certain consults that cannot be rushed, the RACGP said.GP consults are booked by category — A, B, C and D — based on the nature of the concerns to be discussed, and the time likely required for related examination or management.A standard appointment — a level B consult lasting six minutes to 20 minutes — is privately billed at an average of $82. With the MBS rebate of $43.90, the average out-of-pocket cost for patients is about $38.For level C consults up to 40 minutes, and level D consults for anything longer, the bulk-billing incentive does not increase for doctors, who receive the same support funding for a 12-minute consult as they do for a 52-minute consult.This impacts their ability to bulk bill, which is already impacted by location remoteness.“The per-minute value of patient Medicare rebates decreases as patients spend more time with their specialist GP.
