[你懂的] Intuitive Surgical (ISRG) — Can Surgical Robotics Keep Delivering Growth?
While much of Wall Street remains focused on AI chips, data centers, and semiconductor volatility, I want to highlight a different opportunity: Intuitive Surgical (ISRG), a pioneer in robotic-assisted surgery.
What makes this company interesting isn't simply its technology. It's the business model behind it.
Intuitive Surgical sells sophisticated surgical systems, but it also generates recurring revenue from instruments, accessories, and services. As its installed base grows and more procedures are performed, the company has opportunities to generate revenue long after the initial equipment sale.
However, a great business isn't automatically a great investment at every price. With the company approaching another earnings report, investors need to examine whether its growth can justify its valuation.
1. How Does Intuitive Surgical Make Money?
Intuitive Surgical is best known for its da Vinci surgical systems, which allow surgeons to perform procedures using robotic-assisted instruments and advanced visualization technology.
Its business has three main revenue streams:
First, surgical system sales.
Hospitals purchase robotic systems to support surgical procedures. These sales help Intuitive expand its customer base and installed equipment footprint.
Second, instruments and accessories.
Many robotic-assisted procedures require specialized instruments and accessories. As procedure volumes increase, demand for these products can generate recurring revenue.
Third, services and support.
Hospitals need equipment maintenance, technical support, and related services, creating additional revenue opportunities over the life of each system.
This is the key attraction of the business model: revenue does not depend entirely on selling new robots every year.
A larger installed base can support more procedures, which can drive additional demand for instruments, accessories, and services.
In other words, equipment sales help establish the customer relationship, while recurring products and services can generate revenue over time.
2. What Do the Latest Results Tell Us?
According to Intuitive Surgical's second-quarter 2026 earnings release, revenue reached approximately $2.89 billion, representing year-over-year growth of 19%.
Da Vinci procedure volume increased approximately 15% year over year. The company also placed 468 da Vinci systems during the quarter, including 246 of its newer da Vinci 5 systems.
As of June 30, 2026, the global installed base of da Vinci systems stood at approximately 11,710, up 12% year over year.
These figures indicate that the company continues to expand its installed base while procedure volumes remain an important growth driver.
However, there is a critical distinction investors should understand:
Revenue growth of 19% does not guarantee that the same growth rate will continue.
If hospital equipment purchases slow or procedure growth moderates, revenue growth could eventually decelerate.
On the other hand, continued adoption of newer systems and increasing procedure volumes could support further expansion in recurring instrument and service revenue.
For that reason, investors should monitor not only system sales but also procedure growth, installed-base expansion, and the company's ability to convert revenue into profits.
3. Where Does Its Competitive Advantage Come From?
Surgical robotics is not an ordinary hardware business.
Hospitals investing in robotic systems must consider equipment costs, surgeon training, operating-room workflows, maintenance, and long-term clinical outcomes.
Once surgeons become familiar with a platform and hospitals integrate it into their procedures, switching to another system may involve more than simply comparing purchase prices.
Intuitive's competitive advantages therefore extend beyond hardware. They include accumulated clinical experience, surgeon training, established customer relationships, and a supporting service network.
Nevertheless, these advantages are not invincible.
Competitors are developing alternative robotic-assisted surgical systems, while hospitals continue to evaluate equipment costs, operating efficiency, and clinical benefits.
If competing products offer compelling advantages in cost or performance, Intuitive could face greater pricing pressure.
Its long-term success will depend on continuing to demonstrate meaningful clinical and economic value.
4. The Next Catalyst: Quarterly Earnings
Intuitive Surgical is expected to report its third-quarter 2026 results after the U.S. market closes on October 20.
Here are four areas worth watching:
1. Procedure growth
Procedure volume is an important driver of recurring instrument and accessory revenue.
If installed systems continue to increase but procedure growth slows significantly, investors should investigate whether utilization, demand, or market penetration is changing.
2. Adoption of the da Vinci 5 system
Continued adoption of newer systems could support equipment revenue and improve the company's product mix.
However, additional system placements do not automatically translate into proportionate profit growth.
3. Gross margin and operating profitability
Research and development expenses, manufacturing costs, product mix, and competition can all influence profitability.
Investors should determine whether revenue growth is translating into stronger operating earnings.
4. Management's outlook
Forward guidance often matters more to investors than historical results alone.
Even if earnings meet expectations, a weaker outlook for procedure growth could pressure the stock.
Conversely, stronger-than-expected procedure growth and an improved outlook could support a more favorable market assessment.
5. The Biggest Risk: A Great Company Can Still Be Overvalued
One of the biggest risks with Intuitive Surgical is valuation.
Investors often pay a premium for businesses with strong competitive advantages and attractive long-term growth prospects.
But if the share price already reflects years of optimistic expectations, even solid earnings growth may not translate into attractive investment returns.
A second risk is growth deceleration.
As the business expands, Intuitive must continue entering new markets, developing new products, and winning hospital customers. Maintaining high growth becomes more challenging as the business gets larger.
A third risk is competition.
More cost-effective competing systems or changing hospital purchasing decisions could affect Intuitive's market position and profitability.
The takeaway is straightforward: a promising industry does not make every stock within it an automatic buy.
6. My Investment Approach: Earnings First, Valuation Second
I would rather evaluate Intuitive Surgical's latest results before making a decision based on short-term price movements.
If procedure volumes remain healthy, adoption of newer systems continues, and margins and forward guidance remain solid, its long-term growth story deserves further attention.
If growth depends increasingly on equipment sales while procedure growth slows or profitability deteriorates, investors may need to reassess the valuation.
The final step is to compare expected earnings growth with the price investors are paying today.
A company can have excellent technology, an established customer base, and significant long-term opportunities while still delivering disappointing returns if purchased at an excessive valuation.
Final Thoughts: A Promising Healthcare Technology Business, but Price Still Matters
I believe surgical robotics is a sector worth monitoring over the long term, and Intuitive Surgical has a business model that deserves serious attention.
Its appeal goes beyond selling robots. Instruments, accessories, and services create opportunities to build recurring revenue around an expanding installed base.
However, future investment returns will still depend on procedure growth, product competitiveness, profitability, and valuation.
My verdict: ISRG deserves a place on the watchlist, but the priority is not predicting whether the stock jumps after earnings. It is determining whether growth remains sustainable and whether the current valuation offers a reasonable entry point. A great business deserves attention; a sensible price determines whether it deserves an investment.[龇牙]
