$Moderna, Inc.(MRNA)$ MRK ESMO 2026: Intismeran (mRNA-4157) Phase 1 data in pancreatic cancer (PDAC), abstract #3151P.

Phase 1 readout for Intismeran plus FOLFIRINOX in resected PDAC: 20 patients with resected stage IA-III disease. 100% immune response among 19 evaluable patients, with neoantigen-specific T-cell induction across all evaluable patients despite low tumor mutational burden (TMB 0.26–2.75 mut/Mb). TCR sequencing confirmed expansion of new vaccine-induced T-cell clones peaking after FOLFIRINOX completion. KRAS mutations were present in 65% of patients (13/20).

The takeaway here is proof-of-concept that personalized mRNA can ignite T-cell activity in immunologically "cold" tumors like pancreatic cancer. Early Phase 1 signals, but the biological mechanism looks validated.
