I don't see that callback as a big issue. This is more about user mistakes than about the technology.
The user is suppose to stay vigilant through the process and they probably weren't. Anytime you don't use something as it was intended, you can cause problems. That's like holding a gun to your head as a joke thinking it's empty. If it goes off it's your fault not the gun's.
Tesla will probably install some kind of governor that will turn the system off if you're not being vigilant while it's engaged. New technologies always take awhile to get use to.
